Remote monitoring of physiologic parameter(s) (eg, weight, blood pressure, pulse oximetry, respiratory flow rate), initial; set-up and patient education on use of equipment.

Your Practice Will Bill Using CPT CODE- 99454

$ 69 per month

Device(s) supply with daily recording(s) or programmed alert(s) transmission, each 30 days. Can be billed each 30 days, minimum 16 days of monitoring

Your Practice Will Bill Using CPT CODE-99457

$ 52 per month

Remote physiologic monitoring treatment management services, 20 minutes or more of clinical staff/physician/other qualified health care professional time in a calendar month requiring interactive communication with the patient/caregiver during the month Can be billed every 30 days. The 20 minutes of professional time must occur in a calendar month

Your Practice Will Bill Using CPT CODE- 99458

$ 42 per month

New add-on code is for each additional 20 minutes of service.
